There are four (4) different ways to place your, Freedom Of Information Request. Requests are for NHPD-reported incidents only.
- Fill out the LexisNexis FOIA form* (*LexisNexis® Request A Report - is not owned/operated by the City of New Haven)
- Fill out the Request for Incident/Accident/Body Camera Footage form and mail it to:

- Bring the Request for Incident/Accident/Body Camera Footage form to the New Haven Department of Police Service and present it to the Front Desk Sergeant.
- Fax the Request for Incident/Accident/Body Camera Footage form to (203) 946-6274.
Your report request will be processed promptly based on the status of the request queue and the staffing levels of the Records Division.
NHPD Report Fees
- There is a Fifty Cents ($0.50) per page fee for each report requested. Reports 10 pages or less can be emailed free of charge.
- The programming fee for body camera footage requested shall be Fifty Dollars ($50.00) for the first two hours and Twenty-Five Dollars ($25.00) for each additional hour.* Updated 1/1/2024
- The fee for complete accident reconstruction reports is $525.00.
Payments will be accepted in the form of money orders and checks made payable to the Treasurer, City of New Haven.
Please provide as much information as possible when requesting the report or footage:
- Full names of parties involved
- Date of incident
- Time of incident
- Location of incident
- Type of incident (burglary, vandalism, traffic accident, etc.)
- Report number
Please note that the release of information is dictated by the CT State Statute. Some incidents, particularly open investigations, may not be releasable. Some information, also dictated by the CT State Statute, may be redacted from a report or body-worn camera footage.
