Applying for Permits
When submitting an online permit application you will need the following:
- A copy of your non-expired state issued contractor license
- Copy of Workers Compensation/ Liability Insurance. If none please fill out Workers’ Compensation Form – 7B
- Please note form must be notarized!
- Any pertinent plans or floor plans.
- Note: Plans must be clear concise and all measurements should legibly clear if handwritten.
- If you are not able to submit the permit application online you may have an agent submit the permit on your behalf. Permit Applications submitted by the agents will not be accepted without the required "agent letter". (See letter requirements below)
You may create your own letter, it must contain:
- Contractor full name, address, and phone number
- License # (copy must be attached)
- Agents Full Name
- Work Location Address including any specific floors or unit #’s
- Description of work being done (“Per Plans and Specs” – Is Not a Valid Description)
- Must be signed by contractor
